RESOLUTION AUTHORIZING ACCEPTANCE OF A THREE-YEAR GRANT AWARD FROM THE FEDERAL DEPARTMENT OF HEALTH AND HUMAN SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the Federal Department of Health and Human Services has awarded a three-year grant to the Health Services Agency to provide substance abuse treatment in primary care settings through brief physician intervention and case management; and

WHEREAS, this Board has been presented with the form of the Grant Award and has examined and approved it to both form and content and desires to accept the Grant Award:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Request for Proposals process is waived and the President of this Board of Supervisors be, and is hereby authorized and directed to accept said Grant Award for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
