RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO AGREEMENT WITH THE STATE DEPARTMENT OF HEALTH SERVICESTO CONDUCT A CALIFORNIA SAFE COMMUNITIESINJURY PREVENTION PROJECT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an amendment to the agreement with the State of California Department of Health Services, reference is hereby made for further particulars, whereby the State Department of Health Services will provide $21,582.91 to the EMS Program in FY 2001-02 to conduct a California Safe Communities Injury Prevention Project; and

WHEREAS, the Board has been presented with a form of the amendment and has examined and approved it as to both form and content and desires to adopt the Agreement;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board be, and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
