RESOLUTION AUTHORIZING THE EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH CALIFORNIA STATE DEPARTMENT OF REHABILITATION FOR THE BENEFIT PLANNING SERVICES FOR MENTAL HEALTH SOCIAL SECURITY RECIPIENTS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 19, 2001 the board approved an Agreement with California State Department of Rehabilitation for the provision of enhanced benefit planning service for mental health consumers on Social Security benefits for the period of July 1, 2001 through June 30, 2002, Resolution number 64539; and

WHEREAS, this program provides valuable services to maintain employment status and increase self sufficiency of individuals with severe psychiatric disabilities who receive Supplemental Security Income (SSI) and/or Social Security Disability Insurance (SSDI); and

WHEREAS, this Board has been presented with a form of the Amendment to the Agreement with California State Department of Rehabilitation and desires to authorize the execution of the Amendment;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
