RESOLUTION AUTHORIZING AGING AND ADULT SERVICES IN CONJUNCTION WITH THE HEALTH PLAN OF SAN MATEO TO APPLY TO THE STATE OF CALIFORNIA DEPARTMENT OF HEALTH SERVICES FOR THE LONG TERM CARE INTEGRATION DEVELOPMENT PHASE GRANT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, this Board supports Long Term Care Integration, that is, a capitated program that provides the full continuum of integrated medical, social and supportive services to its eligible population; and

WHEREAS, this Board supports the development of a continuum of care that fosters independence and self-reliance, maintains individual dignity and allows consumers of publicly funded long term care services to remain an integral part of their families and community life; and

WHEREAS, this Board encourages the consumers and their families to become partners in developing and implementing the San Mateo County Long Term Supportive Services Project:

NOW THEREFORE, IT IS RESOLVED that this Board authorizes Aging and Adult Services in conjunction with the Health Plan of San Mateo to apply to the State of California Department of Health Services for the Long Term Care Integration Development Phase Grant.
