BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION ACCEPTING CALIFORNIA DEPARTMENT OF HEALTH SERVICES TOBACCO CONTROL SECTION GRANT, WAIVING THE REQUEST FOR PROPOSAL PROCESS FOR CONTRACTED SERVICES DESIGNATED WITHIN THE GRANT AND AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH URSA INSTITUTE AND A FIRST AMENDMENT TO THE AGREEMENT WITH MARTIN L. FORST

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the Human Services Agency, Alcohol and Drug Services, Tobacco Prevention Program, identified a need to provide tobacco prevention interventions targeting young adults in Southern San Mateo County; and

 

WHEREAS, the Human Services Agency, Alcohol and Drug Services, Tobacco Prevention Program submitted a grant application in collaboration with the nonprofit service provider, URSA Institute, and Martin L. Forst, evaluator, to the California Department of Health Services Tobacco Control Section to provide the tobacco prevention services; and

 

WHEREAS, the grant application and notice of intent to award the grant, reference to which is hereby made for further particulars, have been presented to this Board of Supervisors for its consideration and acceptance; and

WHEREAS, the Human Services Agency, Alcohol and Drug Services, Tobacco Prevention Program has now received notice of grant award for a three-year project to fund said services; and

 

WHEREAS, the grant application specifies that URSA Institute will be the service provider for tobacco prevention services funded by the grant and that Martin L. Forst will be the evaluator for said services; and

 

WHEREAS, there has been presented to the Board of Supervisors for its consideration and acceptance an Agreement between the County of San Mateo and URSA Institute and a First Amendment to the Agreement between the County of San Mateo and Martin L. Forst reference to which is hereby made for further particulars; and

 

WHEREAS, this Board has been presented with a form of the Agreements and has examined and approved it as to both form and content and desires to enter into said Agreements:

 

NOW THEREFORE, IT IS HEREBY RESOLVED that the Board of Supervisors: (1) authorizes the Human Services Agency Director to accept said grant award for and on behalf of the County of San Mateo and to execute all necessary documents in connection with the grant, and (2) waives the Request for Proposals process for the contracted services funded by the grant, and authorizes the President of the Board execute the Agreement with URSA Institute and First Amendment to the Agreement with Martin L. Forst for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
