RESOLUTION WAIVING THE REQUEST FOR PROPOSAL PROCESS AND AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH BLUE SHIELD OF CALIFORNIA FOR DENTAL SERVICES TO THE COUNTY FOR THE PERIOD FROM OCTOBER 1, 2002 THROUGH DECEMBER 31, 2005

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, Blue Shield of California has successfully provided County dental services at reduced cost; and

WHEREAS, the County utilized Blue Shield administrative services to provide a network of dentists to administer its self-funded plan; and

WHEREAS, the County elects to waive the request for proposal process and contract with Blue Shield for the period from October 1, 2002 through December 31, 2005; and

WHEREAS, this Board has been presented with a form of the agreement and has examined and approved it as to both form and content and desires to enter into the agreement.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors is authorized and directed to execute an agreement with Blue Shield of California for the provision of dental services and the Clerk of this Board shall attest his signature thereto.
