BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD OF SUPERVISORS TO EXECUTE AN AMENDMENT TO THE GRANT AGREEMENT WITH THE PUBLIC HEALTH INSTITUTE INCREASING THE AMOUNT BY $165,000 AND EXTENDING THE TERM THROUGH AUGUST 31, 2003.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the County was funded by the Public Health Institute to create a plan in collaboration with community partners in South San Francisco and the Coastside community;

WHEREAS, the collaborative plan was approved for implementation by the Public Health Institute;

WHEREAS, the County has been awarded an additional $165,000 in grant funding from the Public Health Institute through August 31, 2003; and

WHEREAS, the Board has bee presented with a form of the amendment to the grant agreement and has examined and approved it as to both form and content and desires to adopt the amendment to the agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and directed to execute said Amendment to the grant Agreement with the Public Health Institute for and on behalf of the County of San Mateo and the Clerk of this Board shall attest the President's signature thereto.
