BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING DIRECTOR OF THE HEALTH SERVICES TO EXECUTE AGREEMENTS AND OTHER DOCUMENTS ON BEHALF OF THE COUNTY OF SAN MATEO IN CONNECTION WITH THE CHILDREN'S HEALTH INITIATIVE

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, this board has committed its support of the Children's Health Initiative, Healthy Kids Project, which will an provide universal health care to all children in San Mateo County, and

WHEREAS, this board has authorized the execution of a Memorandum of Understanding forming the Children's Health Initiative (CHI) Coalition, whereby the Health Services Agency will be the lead organization in the coordination of the Coalition's overall efforts, which will necessitate the County of San Mateo's execution of the underlying agreements and related documents that are necessary to implement the CHI initiative, and

WHEREAS, this board wishes to authorize the Director of Health Services to execute said documents on behalf of the County of San Mateo, subject to the limitations set forth herein.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that in connection with the implementation of the CHI Initiative, the Director of Health Services is authorized to execute on behalf of the County of San Mateo, participation agreements, fund holder agreements, grant applications and agreements whereby the county services to the CHI and receives payment for said services,

NOW THEREFORE, IT IS HEREBY FURTHER DETERMINED AND ORDERED that the Director of Health Services in authorized to execute, on behalf of the County of San Mateo, agreements for the expenditure of county funds in connection with CHI, to the extent that said agreements do not involve the expenditure of more than $25,000 of county funds, and

NOW THEREFORE, IT IS FURTHER DETERMINED AND ORDERED that this board ratifies any such agreements and documents as may have been executed by the Director of Health Services for this purpose prior to the date of adoption of this resolution.
