BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE EPS DIRECTOR TO SIGN A CONTRACT WITH ING FOR THE PROVISION OF LIFE INSURANCE, SUPPLEMENTAL LIFE INSURANCE AND SHORT-TERM DISABILITY BENEFITS FOR EMPLOYEES.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the County offers life insurance, supplemental life insurance and short-term disability benefits to its employees; and

WHEREAS, the County seeks to provide these benefits at the least expensive cost to the County and to employees who obtain supplemental life and disability benefits; and

WHEREAS, the County conducted a Request for Proposal process that solicit proposal from sixty-seven companies, six of which responded; and

WHEREAS, the County has selected ING based on its proposal to replicate current benefits at reduced cost to both the County and employees and wishes the Employee And Public Services Director to sign an agreement upon County Counsel approval;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the proposal from ING is accepted and the Director of Employee and Public Services is hereby authorized and directed to sign an agreement with ING upon approval of its form by County Counsel for and on behalf of the County of San Mateo.
