Agreement with Free at Last

 

Recommendation

Approve a resolution authorizing the President of the Board to execute an agreement with Free At Last to provide outreach, primary care, and case management services to homeless individuals.

 

Background

San Mateo Medical Center was awarded the Health Care for the Homeless (HCH) grant from the Federal Bureau of Primary Health Care of the Health and Resource Services Administration to provide primary care, outreach, health education, case management, and supportive services to the county's homeless population.

HCH is designed to assist homeless adults, families and youth with access to and use of the San Mateo Medical Center continuum of primary care, mental health, substance abuse, and other support services.

 

Discussion

In 2001 the HCH Advisory Committee developed a strategic plan for the HCH Program. The goals of the Strategic Plan include: 1) Serve a large portion of the homeless population who are on the street or in shelters; 2) Provide one or more of the following services to homeless individuals: primary care, dental care, substance abuse and mental health treatment, 3) eliminate barriers and provide better access to services by outreaching to homeless individuals.
A Request for Proposal process followed and based on the priorities of the strategic plan, the HCH Advisory Committee chose to continue to fund three current contractors and provide funding to one new agency. The new agency is Free At Last, which will provide outreach and substance abuse treatment services and will sub-contract with South County Community Health Center to provide primary care services. Free At Last projects to serve 700 homeless individuals in Fiscal Year 2002-03.

County Counsel and Risk Management have approved this agreement.

 

Vision Alignment

This agreement keeps the commitment to Ensure Basic Health and Safety for All and goal number 5: Provide residents access to healthcare and preventive care. The agreement contributes to this commitment and goal by providing the homeless population access to primary care, health education, case management, and supportive services.

 

Fiscal Impact

The term of this agreement is November 1, 2002 through October 31, 2004. The total cost of this agreement for the term is $ 343,950, or $42,993.75 on a quarterly basis for eight quarters. Because all funds for this agreement are provided by the federal HCH grant, there is no net county cost. Expenditures and grant revenues for this agreement are included in the FY2002-03 Hospital and Clinics Adopted Budget and will be included in future requests.
