Agreement with Clara-Mateo Alliance

 

Recommendation

Approve a resolution authorizing the President of the Board to execute an agreement with the Clara-Mateo Alliance to provide outreach, primary care, dental, and case management services to homeless individuals.

 

Background

The San Mateo Medical Center was awarded the Health Care for the Homeless (HCH) grant from the Federal Bureau of Primary Health Care (BPHC) of the Health and Resource Services Administrative to provide primary care, outreach, health education, case management, and supportive services to the county's homeless population.

HCH is designed to assist homeless adults, families and youth with access to and use of the San Mateo Medical Center continuum of primary care, mental health, substance abuse, and other support services.

 

Discussion

In 2001 the HCH Advisory Committee developed a strategic plan for the HCH Program. The goals of the Strategic Plan include: 1) Serve a large portion of the homeless population who are on the street or in shelters; 2) Provide one or more of the following services to homeless individuals: primary care services, dental care services, substance abuse and mental health treatment; 3) Eliminate barriers and provide better access to services by outreaching to homeless individuals.

In April of 2002 the HCH Program submitted a grant application to the BPHC requesting funding to provide primary care, dental, and case management services to the homeless population served by the Clara-Mateo Alliance. In September the BPHC awarded the funding to the HCH Program.

With this funding, Clara-Mateo Alliance will promote early entry into prenatal care for homeless women; provide comprehensive health care and immunizations to homeless children; preventive care for HIV, sexually transmitted diseases and pregnancy, as well as mental health and substance abuse treatment and referrals, for homeless youth and adults; access to comprehensive health care and dental services for homeless adults; and preventive and acute care for elderly homeless adults.

County Counsel and Risk Management have approved this agreement.

 

Vision Alignment

This agreement keeps the commitment to Ensure Basic Health and Safety for All and goal number 5: Provide residents access to healthcare and preventive care. The agreement contributes to this commitment and goal by providing the homeless population access to primary care, dental health, mental health and case management services.

 

Fiscal Impact

The term of this agreement is November 1, 2002 through October 31, 2004. The total cost of this agreement for the term is $334,212 or $13,925.50 on a monthly basis for twenty-four months. Because all funds for this agreement are provided by the federal HCH grant, there is no net county cost. Expenditures and grant revenues for this agreement are included in the FY2002-03 Hospital and Clinics Adopted Budget and will be included in future requests.
