RESOLUTION AUTHORIZING EXECUTION OF A FIRST AMENDMENT TO THE AGREEMENT WITH LESLIE PACKER, PH.D.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, Leslie Packer, Ph.D. has agreed to provide psychological evaluations and courted-ordered therapy for children and families in the Juvenile Court Dependency System; and

WHEREAS, the County Manager's Office has authorized the execution of an Agreement #74200-03-C009 dated June 4, 2002, with Leslie Packer, Ph.D.for such services; and

WHEREAS, the parties desire to amend the Agreement to increase the amount by $2,490 and increase the County's maximum fiscal Obligation to $102,090; and

WHEREAS, this Board has been presented with a form of said amendment and desires to enter into the amended Agreement.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is, and is hereby authorized and directed to execute said amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
