BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AGREEMENTS WITH THE SAN MATEO COUNTY CHILDREN AND FAMILIES FIRST COMMISSION, PENINSULA COMMUNITY FOUNDATION, SAN MATEO HEALTH COMMISSION, AND PENINSULA HEALTH CARE DISTRICT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, There has been presented to this Board of Supervisors for its approval, an Agreement, reference to which is hereby made for further particulars, whereby the San Mateo County Children and Families First Commission will support the Children's Health Initiative by providing funding for health care for children up through age five;

WHEREAS, There has been presented to this Board of Supervisors for its approval, an Agreement, reference to which is hereby made for further particulars, whereby the Peninsula Community Foundation will support the Children's Health Initiative by providing funding to oversee and pay for Health Services Agency and Health Plan of San Mateo activities;

WHEREAS, There has been presented to this Board of Supervisors for its approval, an Agreement, reference to which is hereby made for further particulars, whereby the San Mateo Health Commission will support the Children's Health Initiative by providing health insurance administration;

WHEREAS, There has been presented to this Board of Supervisors for its approval, an Agreement, reference to which is hereby made for further particulars, whereby the Peninsula Health Care District will support the Children's Health Initiative by providing funding for health care for children in its district;

WHEREAS, this Board has been presented with these Agreements and has approved them as to both form and content, and desires to enter into said Agreements;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President's signature thereto.
