RESOLUTION AUTHORIZING THE EXECUTION OF A FIRST AMENDMENT TO THE AGREEMENT WITH THE CALIFORNIA STATE DEPARTMENT OF REHABILITATION AND AUTHORIZING THE DIRECTOR OF THE HUMAN SERVICES AGENCY TO EXECUTE FUTURE AMENDMENTS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on September 11, 2002, the Board of Supervisors adopted Resolution 65539 authorizing the County of San Mateo to develop an Agreement with the State Department of Rehabilitation for the provision of enhanced benefit planning services for mental health consumers on Social Security Benefits for the period of July 1, 2002 through June 30, 2003; and

WHEREAS, it is necessary to Amend this Agreement to include funding for a Benefits Consultant position for the purpose of providing supportive services to program participants; and

WHEREAS, this program provides valuable services to maintain employment status and increase self-sufficiency of individuals with severe psychiatric disabilities who receive Supplemental Security Income (SSI) and/or Social Security Disability Insurance (SSDI); and

WHEREAS, it is expedient, efficient and appropriate for the Human Services Agency Director to execute further amendments to the Agreement to accept any increases in funding amounts or modifications for the completion of the Individual Self Sufficiency Plan; and

WHEREAS, this Board has been presented with a form of this First Amendment to the Agreement with the California State Department of Rehabilitation and desires to enter into same.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said First Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.

BE IT FURTHER RESOLVED that the Director of the Human Services Agency shall be authorized to execute further Amendments to the Agreement to accept funding or Agreement modification.
