RESOLUTION AUTHORIZING EXECUTION OF A FIRST AMENDMENT TO THE AGREEMENT WITH URSA INSTITUTE FOR TOBACCO PREVENTION SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on March 15, 2002, the County of San Mateo entered into an Agreement with URSA Institute for the provision of tobacco prevention services; and

WHEREAS, it is now the mutual desire and intent of the parties to amend the Agreement with URSA Institute; and

WHEREAS, this Board has been presented with forms of the First Amendment to the Agreement with URSA Institute and has examined and approved them as to form and content and desires to enter into said First Amendment to the Agreement.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED, that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said First Amendment to the Agreement with URSA Institute for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
