RESOLUTION AUTHORIZING: 1) THE PRESIDENT OF THE BOARD TO
EXECUTE AMENDMENT A-4 TO THE AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF HEALTH SERVICES; AND 2) THE DIRECTOR OF HEALTH SERVICES TO EXECUTE AMENDMENTS AND MINOR MODIFICATIONS
TO THIS AGREEMENT NOT TO EXCEED $25,000.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance Amendment A-4 to the Agreement, reference to which is hereby made for further particulars, whereby the State of California Department of Health Services shall award AIDS Master Grant Agreement funds to the San Mateo County AIDS Program for implementation of the Evaluating Local Interventions information system; and

WHEREAS, this Board has been presented with Amendment A-4 to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment to the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that Amendment A-4 to the Agreement with the California Department of Health Services is hereby approved, and the President of this Board of Supervisors is hereby authorized and directed to execute the aforesaid Amendment as approved by the County Manager and County Counsel for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President's signature thereto.

BE IT FURTHER RESOLVED, that the Director of Health Services is hereby authorized to execute minor amendments and modifications to the Agreement not to exceed $25,000.
