RESOLUTION AUTHORIZING EXECUTION OF AGREEMENTS

WITH PSYCHIATRISTS ONLY AND J&C NATIONWIDE

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance Agreements, reference to which are hereby made for further particulars, whereby Pychiatrists Only and J&C Nationwide shall provide locum tenens services for the Mental Health Services Division for a collective total amount of $120,000; and

WHEREAS, this Board has been presented with the Agreements and has examined and approved them as to both form and content and desires to enter into the Agreements;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby, authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto;

BE IT FURTHER RESOLVED that the Director of Health Services is hereby authorized to execute minor amendments and modifications up to an aggregate of $25,000 for the term of the agreement.
