RESOLUTION AUTHORIZING EXECUTION OF A FIRST AMENDMENT TO THE AGREEMENT WITH URBAN WILDLIFE MANAGEMENT, INC. AND AUTHORIZING THE DIRECTOR OF HEALTH SERVICES TO EXECUTE AMENDMENTS AND MINOR MODIFICATIONS UP TO $25,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment, reference to which is hereby made for further particulars, whereby the Aging and Adult Services Division shall provide a Public Guardian/Conservatorship Program, which serves frail elderly, physically and/or mentally disabled residents who are substantially unable to provide for their own personal needs of health, food, clothing or shelter and/or are unable to manage financial resources or resist fraud or undue influence;

WHEREAS, Urban Wildlife Management, Inc. shall provide residential clean-up for clients who are conserved under the responsibility of the San Mateo Public Guardian; and

WHEREAS, this Board has been presented with the Amendment and has examined and approved it as to both form and content and desires to enter into the Amendment:

NOW, THEREFORE, IT IS RESOLVED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment with Urban Wildlife Management, Inc. for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto; and

BE IT FURTHER RESOLVED, that the Director of Health Services is hereby authorized to execute subsequent Amendments and minor modifications to the Agreement with Urban Wildlife Management, Inc., not to exceed the aggregate of $25,000 for the term of the agreement.
