RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT
WITH THE SAN MATEO HEALTH COMMISSION

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby the Health Plan of San Mateo will provide San Mateo Medical Center an additional $1,000,000 to fund activities for the Medical Center related to Obstetric (OB) Services, comprehensive Diabetes management, and the management of inpatient costs; and

WHEREAS, San Mateo Medical Center shall work with San Mateo Health Commission to improve access to and cost effectiveness of OB services for San Mateo County Medi-Cal beneficiaries including the possible addition of OB services at San Mateo Medical Center and at the various clinic locations; and

WHEREAS, with San Mateo Health Commission, San Mateo Medical Center shall establish a system for transferring inpatients from non-contracting hospitals that is medically appropriate and cost-effective; and

WHEREAS, San Mateo Medical Center shall establish a comprehensive Diabetes case management program to assist San Mateo Health Commission in improving member outcomes and in meeting objectives of state required Diabetes initiative; and

WHEREAS, San Mateo Medical Center shall work with San Mateo Health Commission to develop and implement procedures to treat chronic care conditions (including pain management) in a medically appropriate and cost effective manner; and

WHEREAS, San Mateo Medical Center shall work with San Mateo Health Commission to develop and implement a plan to reduce the number of inpatient days for medical services which, from a medically appropriate perspective, could be provided on an outpatient basis; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
