RESOLUTION AUTHORIZING A SECOND AMENDMENT TO THE AGREEMENT WITH YOUTH AND FAMILY ASSISTANCE (YFA), A FIRST AMENDMENT TO THE AGREEMENT WITH FAMILY AND COMMUNITY ENRICHMENT SERVICES (FACES), A SUPERCEDING AGREEMENT WITH YOUTH AND FAMILY ENRICHMENT SERVICES (YFES), AND AUTHORIZING THE DIRECTOR OF HEALTH SERVICES TO EXECUTE AMENDMENTS AND MINOR MODIFICATIONS TO THE YFES AGREEMENT NOT TO EXCEED THE AGGREGATE OF $25,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a second Amendment to the Agreement with YFA, reference to which is hereby made for further particulars, whereby YFA shall provide mental health services in the Girls’ Juvenile Drug Court Expansion program, substance abuse services, outpatient mental health services, and youth development initiative services, and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a first Amendment to the Agreement with FACES, reference to which is hereby made for further particulars, whereby FACES shall provide mental health services in the Girls’ Juvenile Drug Court Expansion program, Public Health Community Worker services to Prenatal to Three Initiative (Pre-to-Three), Pre-to-Three mental health and substance abuse services for Medi-Cal eligible infants and their families, and outpatient mental health services for the Mental Health Plan (MHP); and

WHEREAS, this Board has been presented with the form of the Amendments to the Agreements with YFA and FACES and has examined and approved them as to both form and content and desires to enter into the Amendments to the Agreements; and

WHEREAS, YFA and FACES have merged, effective July 1, 2003, to become Youth and Family Enrichment Services (YFES); and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an agreement, reference to which is hereby made for further particulars, whereby YFES shall provide mental health services in the Girls’ Juvenile Drug Court Expansion program, substance abuse services, outpatient dual-diagnosis services as authorized by the Mental Health Plan, Community Worker services to Pre-to-Three, Mental Health Outpatient services for the Mental Health Plan, and youth development services; and

WHEREAS, this Board has been presented with the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby, authorized and directed to execute said Amendments to Agreements with YFA and FACES, and said Agreement with YFES for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Director of Health Services is hereby authorized to execute subsequent Amendments and minor modifications to the Agreement with YFES, not to exceed the aggregate of $25,000.
