Recommendation |
Adopt a resolution authorizing the President of the Board to execute an agreement with the City and County of San Francisco for acceptance of Ryan White CARE Act funds for fiscal year 2003-04. |

Background |
The purpose of the CARE Act is to provide emergency assistance to eligible metropolitan areas (EMAs) that are disproportionately affected by the HIV epidemic and to make financial assistance available for the development, organization, coordination, and operation of more effective and cost efficient systems for the delivery of essential services to individuals and families with HIV disease. |

Discussion |
CARE Act funding comes to the San Mateo County AIDS Program (SMCAP) through an agreement with the City and County of San Francisco. The amount of funding San Mateo County receives each year is determined by the San Francisco Ryan White CARE Act Planning Council. The CARE Act award for March 1, 2003 through February 28, 2004 is $2,411,008. |

The CARE Act offsets the cost of a total of 16.9 FTE positions providing direct services to clients with HIV/AIDS. These positions provide interdisciplinary staffing for the AIDS Program Clinic, Client Services Unit, Health Outreach Team, and Client Socialization Center. The funds also pay for medical supplies, laboratory costs, office supplies, and the following contract services: dental care, housing assistance, food services, mental health services, substance abuse treatment, medical specialty services, dietitian services, medical transcription, attendant care, and basic client support. |

Vision Alignment |
The contract with the City and County of San Francisco keeps the commitment of ensuring basic health and safety for all and goal number 5: Residents have access to healthcare and preventive care. It also keeps the commitment of goal number 8: Help vulnerable people - the aged, disabled, mentally ill, at-risk youth and others - achieve a better quality of life. The contract contributes to these commitments and goals by providing housing, dental, food, and mental health services to improve the quality of life of those living with HIV/AIDS. |

Fiscal Impact |
The term of this agreement is March 1, 2003 through February 28, 2004. The $2,411,008 in revenue and associated expenses is included in the recommended FY 2003-04 AIDS Program budget. All funding for this contract, including administrative costs, is provided through the CARE Act grant. A maintenance of County effort is required for all CARE funding which amounts to the net County cost for the AIDS Program for FY 2003-04, i.e., approximately $373,000. |
