Resolution waiving the bid process and authorizing the purchasing agent to enter into a 60-month purchase agreement with ScriptPro, to provide a robotic pharmacy dispensing system.

 

Recommendation

Approve a Resolution waiving the bid process and authorizing the purchasing agent to enter into a 60-month purchase agreement with ScriptPro, to provide a robotic pharmacy dispensing system.

 

Background

The recent Joint Commission on Accreditation of Healthcare Organizations (JCAHO) of FY2002 focused on medication errors and patient safety and San Mateo Medical Center (SMMC) received a Type one in Medication Use with five areas of non compliance. SB 1875 passed by the California State Legislator requires every general acute care hospital to adopt a formal plan for minimizing medication-related errors as a condition of licensure.

Since 1995, the activity of the pharmacy has grown specifically in the outpatient area (6,000 scripts per month to 10,000). Pharmacy costs have also escalated in double digit increases until this past budget year. The pharmacy was able to take advantage of the 340B program for the 39th Ave clinic and this stabilized cost increases for the current budget year.

 

Discussion

ScriptPro is a robotic system that automatically selects a prescription vial of the proper size, fills the vial with the proper drug, print and apply the correct prescription label to the vial, and deliver the filled vial, uncapped, to the pharmacist for verification.

The system will store and dispense from 200 universal drug cells, available in 40 cubic inch and 120 cubic inch capacities that recalibrate on site. Bar codes confirm correct drug product and correct drug strength. The system fills vials directly from the drug storage cell, preventing cross contamination. The prescription fills up to 120 prescriptions per hour. It interfaces with the pharmacy host computer system.

ScriptPro occupies no more than 30 square feet and operates on standard 110 V, 60 Hz and 15A power. Communication is an analog RJ-11 telephone outlet accessible to an outside line and capable of receiving incoming calls.

Baker and ScriptPro are the only two providers of pharmacy robotic systems nationwide. ScriptPro differs from the other robotic system (Baker Autoscript III-A) in that the drug cassettes hold 40 cubic inches for single cells, 120 cubic inches for double cells versus 16.5 cubic inches in the Baker model. ScriptPro holds 200 drugs versus 150 drugs. ScriptPro cells recalibrate on site when changing drugs with different NDC numbers while the cells for the Baker system must be sent back to the company for re-calibration. ScriptPro counting occurs directly from the drug cell preventing no cross contamination while the Baker Autoscript counts directly through a main channel setting up the possibility of cross contamination. In the Baker system, reports cannot be run while the robot is in use. This is not the case with ScriptPro. The Baker system requires 220V/3 phase power and compressed air 100 psi or higher. ScriptPro requires only a dedicated 20V, 15A, 60 Hz electrical outlet. Due to these differences, ScriptPro is the product that meets the pharmacy needs.

ScriptPro will assist San Mateo Medical Centers' Pharmacy in meeting its plan to eliminate or substantially reduce medication errors as was submitted to the Department of Health Services as mandated by SB1875.

 

Vision Alignment

This agreement keeps the county's commitment of responsive, effective and collaborative government, and goal number 21, County employees understand, support and integrate the County vision and goals into their delivery of services. This agreement contributes to this commitment and goal by providing a system which will minimize medication related errors.

 

Fiscal Impact

The term of this Agreement is five years, from August 1, 2003 to July 31, 2008. Contractor will be paid an initial payment of $6,420, followed by a monthly service fee of $3,210 and a monthly support fee of $465 for 60 months. Monthly fees include costs for training and implementation. The maximum amount of the agreement including applicable taxes is $245,641. Funds were included in the San Mateo Medical Centers FY2003-04 Adopted Budget and will be included in future year Budget Requests.
