RESOLUTION APPROVING AMENDMENT 2 TO AGREEMENTS WITH CATHOLIC CHARITIES OF SAN FRANCISCO, MILLS-PENINSULA SENIOR FOCUS, INC., PENINSULA VOLUNTEERS/ROSENER HOUSE, AND CITY OF SOUTH SAN FRANCISCO ADULT DAY CARE PROGRAM

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance Amendment 2 to the Agreements, reference to which is hereby made for further particulars, whereby Catholic Charities of San Francisco, Mills-Peninsula Senior Focus, Inc., Peninsula Volunteers/Rosener House, and City of South San Francisco Adult Day Care Program shall provide adult day care services for MSSP clients;

WHEREAS, the maximum the County shall be obligated to pay collectively for the period July 1, 2003 through June 30, 2004 to all Contractors for services provided through Agreements approved by Resolution number 64534 dated June 19, 2001 is $120,000; and

WHEREAS, this Board has been presented with the Amendments to the Agreements and has examined and approved them as to both form and content and desires to enter into the Amendments to the Agreements:

NOW, THEREFORE, IT IS RESOLVED that Amendment 2 to the Agreements with Catholic Charities of San Francisco, Mills-Peninsula Senior Focus, Inc., Peninsula Volunteers/Rosener House, and City of South San Francisco Adult Day Care Program are hereby approved, and the President of this Board of Supervisors is hereby authorized and directed to execute the aforesaid Amendments for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President's signature thereto.
