IN-HOME SUPPORTIVE SERVICES (IHSS) PUBLIC AUTHORITY, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING AMENDMENT NO. 2 WITH IN-HOME SUPPORTIVE SERVICES PUBLIC AUTHORITY AND ADDUS HEALTHCARE, INC.

RESOLVED, by the IHSS Public Authority of the County of San Mateo, State of California, that

WHEREAS, the Board of Supervisors of San Mateo has designated itself as the Governing Board of the San Mateo County IHSS Public Authority to carry out programs pursuant to the In-Home Supportive Services program;

WHEREAS, there has been presented to this Governing Board for its consideration and acceptance Amendment No. 2 to the Agreement, reference to which is hereby made for further particulars, whereby Addus HealthCare, Inc., shall provide In-Home Supportive Services to clients of Aging and Adult Services; and

WHEREAS, this Governing Board has been presented with a form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that Amendment No. 2 to the Agreement with Addus HealthCare, Inc., is hereby approved, and the President of this Governing Board is hereby authorized and directed to execute the aforesaid Amendment for and on behalf of the In-Home Supportive Services Public Authority, and the Clerk of this Board shall attest the President's signature thereto.
