RESOLUTION AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH THE CALIFORNIA STATE DEPARTMENT OF REHABILITATION FOR THE BENEFIT PLANNING SERVICES FOR MENTAL HEALTH SOCIAL SECURITY RECIPIENTS AND AUTHORIZING THE HUMAN SERVICES AGENCY DIRECTOR TO EXECUTE FUTURE AMENDMENTS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the County of San Mateo has developed an Agreement with the State Department of Rehabilitation for the provision of enhanced benefit planning services for mental health consumers on Social Security Benefits for the period of October 1, 2003 through June 30, 2004; and

WHEREAS, this program provides valuable services to maintain employment status and increase self-sufficiency of individuals with severe psychiatric disabilities who receive Supplemental Security Income (SSI) and/or Social Security Disability Insurance (SSDI); and

WHEREAS, As it is expedient, efficient and appropriate for the Human Services Agency Director to execute future Amendments to the Agreement to accept increases in funding amounts not to exceed $25,000 or modifications for the completion of the Individual Self Sufficiency Program, the Human Services Agency is requesting that the Board of Supervisors authorize the Director of Human Services Agency to execute future Amendments; and

WHEREAS, this Board has been presented with a form of the Agreement with the California State Department of Rehabilitation and desires to authorize the execution of the Agreement.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.

BE IT FURTER DETERMINED AND ORDERED that the Human Services Agency Director be authorized to execute future Amendments for the purpose of accepting increases to funding and modifications.
