RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT WITH CALIFORNIA DEPARTMENT OF HEALTH SERVICES AND SECOND AMENDMENTS TO THE AGREEMENTS WITH THE URSA INSTITUTE AND MARTIN L. FORST

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, On March 22, 2002, the Board of Supervisors accepted a three year grant award from the California Department of Health Services, to provide tobacco prevention services, waived the request for proposals process, and entered into Agreements with nonprofit services provider URSA Institute and evaluator Martin L. Forst; and

WHEREAS, the Human Services Agency has received notice of a Grant Amendment from The California Department of Health Services which will increase funding and extend the term of the Grant to June 30, 2005; and

WHEREAS, the Amendment has been presented to this Board of Supervisors for its consideration and acceptance; and

WHEREAS, the Grant Amendment specifies that URSA Institute is the service provider for tobacco prevention services and Martin L. Forst is the evaluator for said services; and

WHEREAS, there has been presented to the Board of Supervisors for its consideration and acceptance of a Second Amendment to the Agreement between the County of San Mateo and URSA Institute for tobacco prevention services, and a Second Amendment to the Agreement between the County of San Mateo and Martin L. Forst for evaluation services; and

WHEREAS, this Board has been presented with forms of the two Second Amendments and has examined and approved them both as to form and content and desires to enter into said Agreements:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED, that the Board of Supervisors: (1) authorizes the President of the Board of Supervisors to execute an amendment to the Grant Agreement with The California Department of Health Services Tobacco Control Section and all necessary documents in connection with the amendment, (2) and authorizes the President of the Board to execute Second Amendments to the Agreements with URSA Institute and Martin L. Forst for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
