RESOLUTION AUTHORIZING THE EXECUTION OF A SECOND AMENDMENT TO THE AGREEMENT WITH DELOITTE CONSULTING

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the Board of Supervisors approved an Agreement with Deloitte Consulting on March 14, 2003, to initiate the design, development and implementation of the One-E-App, a web-based application processing system that facilitates the processing, collection and storage of data for the Healthy Kids and other locally managed eligibility programs; and

WHEREAS, the Board of Supervisors approved a First Amendment on May 13, 2003 to increase the amount of the Agreement for the purpose of adding Phase II, to expand the existing system to include additional managed health programs; and

WHEREAS, both the County and the Contractor wish to further amend the Agreement to increase the amount for the purpose of enhancing the One-E-App system by adding a paper renewal form option and provide data entering capabilities; and

WHEREAS, this Board has been presented with a form of such Second Amendment to the Agreement and said Board has examined and approved it as to both form and content and desires to enter into same.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Second Amendment to the Agreement for and on behalf of the County of San Mateo; and the Clerk of this Board shall attest the President's signature thereto; and
