RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO AN AGREEMENT WITH WORKFORCE MEDICAL GROUP, INC. FOR OCCUPATIONAL HEALTH SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the County must provide pre-placement and occupational health services to its employees to comply with Federal and State mandates;

WHEREAS, the County previously entered into an agreement with Workforce Medical Group in September 2001 to provide occupational health services;

WHEREAS, there has been an increase in the usage of the services of Workforce Medical Group and the existing contract amount is insufficient to cover the continued costs of those services;

WHEREAS, there has been presented to this Board of Supervisors a proposed amendment to the agreement for its consideration and acceptance reference to which is hereby made for further particulars; and;

WHEREAS, this Board of Supervisors has examined and approved the amendment to the agreement as to form and content and desires to enter into same;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to, execute said amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
