Recommendation |
Adopt a resolution authorizing the President of the Board to execute an agreement with the State Department of Health Services to provide immunization assistance for FY 2003-04. |
|
Background |
San Mateo County has received funding from the State Department of Health Services for immunization assistance each year since the mid-1970s. This funding allows the Immunization Assistance Program (IAP) staff to plan and coordinate immunization outreach, education, and service delivery countywide. IAP staff distribute State-supplied vaccines to public clinics; conduct annual reviews of student immunization records; coordinate and conduct educational programs and distribute periodic immunization updates to community health providers; support development of the Bay Area Regional Immunization Registry; arrange for and conduct immunization clinics in response to identified need; and offer immunization information and referral services to the general public. The staff also monitor immunization providers and offer quality assurance and technical assistance. |

Discussion |
The IAP staff will be involved in the following activities to improve immunization coverage in San Mateo County: |

Sponsor a County-wide training for nurses on vaccine administration, storage and handling |
· |
Work with the San Mateo Medical Center (SMMC) to improve immunization delivery by removing as many barriers to service as possible |
· |
Provide vaccine and patient information materials to SMMC |
· |
Offer additional immunization clinics in under-served areas |
· |
Provide training to childcare providers on immunization requirements and documentation |
· |
Host at least one immunization teleconference for public and private sector medical providers |
· |
Work with Women, Infants and Children (WIC) staff to educate parents about the immunization schedule and to assess their children's immunization records for up-to-date status |
· |
Support the Bay Area Regional Immunization Registry |

The Bay Area Regional Registry will improve immunization rates by providing a consistent reminder/recall system to ensure timely immunizations. It will also reduce the number of over-immunized children. The benefits will increase as the number of participating physicians increases, so the impact will not be immediate. |

Program Indicators |
A review of the available data indicates that the percent of immunization completion remains fairly constant. A significant portion of this is due to a lack of adequate technology to accurately measure the actual immunization rate in real time. The data is collected manually with the indicator counted retrospectively at age 5, when a child enters Kindergarten. Because of this method, reporting of actual numbers is delayed by three years. |

Vision Alignment |
The agreement with the State Department of Health Services keeps the commitment of ensuring basic health and safety for all and goal number 6: Children grow up healthy in safe and supportive homes and neighborhoods. The agreement with the State Department of Health Services contributes to this commitment and goal by providing funds to support immunization activities in San Mateo County, thereby increasing the likelihood of children growing up healthy. |

Fiscal Impact |
The term of the agreement with the State Department of Health Services is July 1, 2003 through June 30, 2004. The recommended FY 2003-04 budget for Public Health includes $200,024 in revenue and expenditures for this program. $44,000 of this funding is for a subcontract with the Ravenswood Family Health Center to support and enhance immunization services in the South County area. Net County cost for this program is $37,486. |
