RESOLUTION AUTHORIZING AMENDMENT 1 TO AGREEMENT NO. MS-0304-13 WITH THE CALIFORNIA DEPARTMENT OF AGING FOR THE CONTINUATION OF THE MULTI PURPOSE SENIOR SERVICES PROGRAM (MSSP) FOR FISCAL YEAR 2003-04

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment, reference to which is hereby made for further particulars, whereby State funds provided to the County are increased for the Multipurpose Senior Services Program that provides social and health support necessary to enable frail elderly to remain in their own homes; and

WHEREAS, this Board has been presented with the Amendment to Agreement MS-0304-13 and has examined and approved it as to both form and content and desires to enter into the Amendment to the Agreement; and,

WHEREAS, the parties now wish to amend the Agreement to provide a Multipurpose Senior Services Program and to increase the amount by $107,493 for a total obligation of $739,385 without a change in the term;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute Amendment 1 to Agreement No. MS-0304-13 for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President's signature thereto.
