RESOLUTION AUTHORIZING THE EXECUTION OF A SECOND AMENDMENT TO THE AGREEMENT WITH DELOITTE CONSULTING TO DEVELOP OF THE HOMELESS MANAGEMENT INFORMATION SYSTEM (HMIS)

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on October 29, 2002 the Board of Supervisors authorized the execution of an Agreement with Deloitte Consulting for the purpose of developing a web-based system that would provide an easier and more accurate process of data entry by treatment providers of the Alcohol and Other Drugs Services program; and

WHEREAS, on June 24, 2003 the Board of Supervisors executed a First Agreement for the purpose of transitioning the system over to County for system maintenance; and

WHEREAS, the parties now wish to further amend the Agreement to develop the Homeless Management Information System (HMIS); to increase the amount by $375,000 for a total obligation of $1,844,005 and extend the term to December 1, 2004; and

WHEREAS, this Board of Supervisors has been presented with a form of the Second Amendment to the Deloitte Consulting Agreement and said Board has examined and approved same as to both form and content and desires to enter into same.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute the Second Amendment to the Deloitte Consulting Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
