San Mateo County Central Labor Council Community Access Program and Children's Health Initiative Grant Contract

 

Recommendation

Adopt a resolution authorizing the President of the Board to execute an agreement with the San Mateo County Central Labor Council.

 

Background

The San Mateo County Central Labor Council has partnered with the Health Services Agency since May 2001 to participate in efforts of the federal Community Access Program (CAP), which provides communities with infrastructure-building funding to develop innovative programs that create a more efficient, cost-effective, and coordinated system of health care leading to expanded access for uninsured and underserved populations. More recently, the Labor Council has partnered with Health Services on the San Mateo County Children's Health Initiative (CHI), which aims to provide health insurance coverage to all county children.

 

Through its Health@Work project, the Labor Council has been successful in creating a formal mechanism to address health and health care access issues facing its more-than-60,000 members in the various union locals. For its low-income workers and their uninsured children, it has developed innovative chronic disease education and management programs and strategies to increase health insurance coverage. The Health@Work's program has been vital in responding to the capacity constraints of the community health care system. The program has enabled union members to proactively care for potential or existing chronic conditions, such as diabetes, thereby reducing clinic visits and costly hospitalizations.

 

Discussion

In September 2003, the Health Services Agency and the San Mateo Labor Council were awarded a third year of CAP grant funding. This CAP grant program will build on CHI's success and increase access to and use of preventive and primary care options for children. Specifically, Health@Work will work with its union members on education and management programs that reduce chronic conditions among children and families.

 

In addition, Health@Work was awarded a second year of CHI funding to provide outreach, enrollment and retention services to its members for Medi-Cal, Healthy Families and Healthy Kids. The CHI Coalition reduced Health@Work's year 2 grant level by $5,000 and reallocated these resources to other organizations, which successfully reached a greater number of uninsured children. Due to this reduction in CHI funding, the Health@Work calendar year (CY) 2004 performance objective will remain at the same level as the previous year.

 

The San Mateo County Labor Council's work will continue to assist the county in developing a more coordinated and community-oriented system of care. In addition, its work will expand health insurance coverage and curtail the widespread growth of chronic diseases, leading to overall health care cost savings.

Vision Alignment

The Labor Council contract keeps the commitment of Ensuring Basic Health and Safety for All and goal number 5: Residents have access to health care and preventive care. The Labor Council contributes to this commitment and goal by providing improved access to health care and increased health coverage for all its local union members.

 

Fiscal Impact

The maximum amount of this agreement is $114,550 for the term September 1, 2003 to December 31, 2004. $81,550 will come from CAP funding (September 1, 2003-August 31, 2004); $25,000 will come from the Children's Health Initiative (December 1, 2003-December 31, 2004), which is a mix of public and private dollars; and $8,000 will come from the United Way via the Peninsula Community Foundation. This contract is late due to a delay in receiving notification of funding from CAP. Funding and expenditures are included in the Health Services Administration FY 2003-04 and FY 2004-05 budgets. There is no net county cost associated with this agreement.
