RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH STATE DEPARTMENT OF HEALTH SERVICES FOR THE CHILDREN'S DENTAL DISEASE PREVENTION PROGRAM

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an amendment to an agreement, reference to which is hereby made for further particulars, whereby the State of California Department of Health Services will provide an additional $1,277 for the Dental Disease Prevention Program; and

WHEREAS, the term of the agreement shall remain July 1, 2001 through June 30, 2004 and the revised total amount of funding is $74,514; and

WHEREAS, this Board has been presented with a form of the Amendment and has examined and approved it as to both form and content and desires to enter into the Amended Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board hereby authorizes the President of this Board of Supervisors to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.

BE IT FURTHER RESOLVED, that the Director of Health Services is hereby authorized to execute minor amendments and modifications to the Agreement not to exceed $25,000.
