RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH PER DIEM STAFFING SYSTEMS, INC.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, it is necessary to contract with respiratory therapist to provide San Mateo Medical Center with access to respiratory staff available to fill in for vacancies when necessary and to maintain maximum access to the available work force; and.

WHEREAS, on May 12, 2003, the Chief Executive Officer of San Mateo Medical Center approved an Agreement with Per Diem Staffing Systems, Inc., subsequently amended on January 1, 2004; and

WHEREAS, due to vacancies and shortages in the Respiratory Department, it is necessary to increase the maximum amount of the Agreement by $70,000 from $55,000 to $125,000; and

WHEREAS, the term of the Agreement remains the same, April 1, 2003 to March 31, 2004; and

WHEREAS, there have been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement, reference to which is hereby made for further particulars, whereby Per Diem Staffing Systems, Inc. shall provide temporary respiratory staffing to San Mateo Medical Center; and

WHEREAS, this Board has been presented with a form of the Amendment to the Agreement and has examined and approved them as to both form and content and desires to enter into the Amendment to the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
