RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH THE CITY OF BERKELEY HEALTH AND HUMAN SERVICES DEPARTMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, it has been presented to this Board of Supervisors for its consideration and acceptance an agreement with the City of Berkeley, reference to which is hereby made for further particulars, whereby the Robert Wood Johnson Foundation will provide funding for a joint program, entitled "Active for Life", in the amount of $972,460, and

WHEREAS, San Mateo County will in turn contract with the City of Berkeley to implement and reach one third of the program goals and therefore provide the City with $294,000 of the funding; and

WHEREAS, the term of this agreement is February 1, 2003 through December 31, 2006, unless the Robert Wood Johnson Foundation funding is terminated at an earlier date; and

WHEREAS, this Board has been presented with a form of the agreement and has examined and approved it as to both form and content and desires to enter into said Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and directed to execute the Contract for and on behalf of the County of San Mateo and the Clerk of this Board shall attest the President's signature thereto.
