RESOLUTION AUTHORIZING AMENDMENT 1 TO AGREEMENT NO. FF-0304-08 FOR TITLE III/VII WITH THE CALIFORNIA DEPARTMENT OF AGING (CDA) FOR FISCAL YEAR 2003-2004

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment, reference to which is hereby made for further particulars, whereby Federal and State funds provided to the County are increased for Title III/VII for the continuation of Supportive Services, Ombudsman, Congregate Nutrition, Home Delivered Meals, Preventive Health and Elder Abuse programs necessary to enable frail elderly to remain in their own homes;

WHEREAS, this Board has been presented with the Amendment to Agreement FF-0304-08 and has examined and approved it as to both form and content and desires to enter into the Amendment of the Agreement; and

WHEREAS, the parties now wish to amend the Agreement to provide Supportive Services, Ombudsman, Home Delivered Meals, Congregate Nutrition, and Preventive Health and Elder Abuse programs and increasing the amount by $324,433 for a total obligation of $2,737,112 without a change in the term:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute Amendment 1 to Agreement No. FF-0304-08 for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President's signature thereto.
