RESOLUTION AUTHORIZING THE EXECUTION OF A SECOND AMENDMENT TO THE AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF HEALTH SERVICES (CDHS) FOR TOBACCO CONTROL YOUTH COALITION SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on March 13, 2001, the Board of Supervisors adopted a resolution authorizing the Director of the Human Services Agency to accept a California Department of Health Services (CDHS) grant, funded by the American Legacy Foundation and to execute all necessary documents in connection with the grant, for the period of February 1, 2001 through August 31, 2001; and

 

WHEREAS, on November 20, 2001, the Board of Supervisors adopted a resolution authorizing the Director of Human Services Agency to accept the Amendment to the Agreement with CDHS for the period of September 1, 2001 through August 31, 2003; and

 

WHEREAS, the parties desire to increase the amount of the Agreement by $59,500 for a total obligation of $204,500 in order to continue youth tobacco control youth coalition services in San Mateo County for the period September 1, 2003 through February 28, 2005; and

 

WHEREAS, the activities included in this Second Amendment incorporates a youth development model to build the skills of youth, give teens decision-making power, and offer youth meaningful opportunities for involvement in a safe, positive environment; and

 

WHEREAS, this Board has been presented with a form of such Second Amendment and said Board has examined and approved same as to both form and content and desires to enter into the same.

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Second Amendment for and on behalf of the County of San Mateo; and the Clerk of this Board shall attest the President's signature thereto.
