RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH LUCILE SALTER PACKARD CHILDREN’S HOSPITAL

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, Lucile Salter Packard Children’s Hospital has experience in the community, excellent training programs, and past partnerships in providing consultative services regarding women’s and children’s services; and

WHEREAS, Lucile Salter Packard Children’s Hospital will provide consultative assistance and tertiary partnership for the planning of women’s and children’s services at San Mateo Medical Center; the term of the agreement is March 15, 2004 to March 14, 2006; and

WHEREAS, the term of the agreement with Lucile Salter Packard Children’s Hospital is March 15, 2004 to March 14, 2006 and the maximum amount of the agreement is $130,000; and

WHEREAS, this Board has been presented with a form of the agreement and has examined and approved it as to both form and content and desires to enter into the agreement::

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
