SUBJECT:

Amendment to the Grant Agreement with the Public Health Institute.

 

Recommendation

    Adopt a resolution authorizing the President of the Board to execute an amendment to the grant agreement with the Public Health Institute.

 

Background

On November 28, 2000, your Board adopted a resolution accepting a grant from the Public Health Institute in the amount of $150,000 to partner with the Coastside community and the City of South San Francisco to implement the Partnership for the Public’s Health (PPH). Your Board has approved amendments to this agreement to extend the end date to August 31, 2004, and to accept an additional $495,000 in funding for an agreement total of $645,000.

 

The Partnership for the Public’s Health is an extension of the Community Oriented Health Systems (C.O.H.S.) pilot of an alternative system of healthcare delivery in our communities. Over the four year term of this agreement, the partnerships with the Coastside community and the City of South San Francisco have been very successful in improving healthcare for their residents.

 

Discussion

This Amendment No. 5 extends the PPH grant to December 31, 2004 to allow San Mateo County to complete close-out activities. No additional funds will be awarded, however, San Mateo County has been permitted to expend remaining funds during this period.

 

Vision Alignment

The amendment to the grant agreement with the Public Health Institute keeps the commitment of Ensuring Basic Health and Safety for All and goal number 8: Help vulnerable people – the aged, disabled, mentally ill, at-risk youth and others – achieve a better quality of life. The grant agreement contributes to this commitment and goal by working with communities to establish health-related priorities and develop strategies for addressing these priority areas.

 

Fiscal Impact

This amendment does not change the total award amount of $645,000. Funding for close-out activities for this grant will come from roll-over funding from the grant award and total $17,422. This funding has been included in the Health Services Administration budget for FY 2004-05.
