RESOLUTION AUTHORIZING AGREEMENT NUMBER 03-76075 WITH THE STATE DEPARTMENT OF HEALTH FOR THE AIDS MEDI-CAL WAIVER PROGRAM

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement with the State Department of Health Services to provide case management, skilled nursing, attendant care, psycho-social counseling, durable medical equipment, minor modifications to homes, and transportation services to clients of the AIDS Medi-Cal Waiver Program; and

WHEREAS, both parties now wish to enter into an Agreement to provide AIDS Medi-Cal Waiver Program services for the period of January 1, 2004 through December 31, 2006, for a total obligation of an annual cost cap per AIDS Waiver client of $13,209:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that Agreement Number 03-76075 with the State Department of Health for the AIDS Medi-Cal Waiver Program is hereby approved, and the President of this Board of Supervisors is hereby authorized and directed to execute the aforesaid Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
