RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE
AMENDMENT A-5 TO THE AGREEMENT WITH THE CALIFORNIA DEPARTMENT
OF HEALTH SERVICES FOR THE STATE AIDS MASTER GRANT
AGREEMENT FOR FISCAL YEARS 2001-04

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement with the California Department of Health Services (CDHS) reference to which is hereby made for further particulars, whereby San Mateo County AIDS Program will provide Early Intervention, HIV prevention and education, high risk testing and outreach, case management and a youth social marketing campaign program; and

WHEREAS, the term of the Agreement remains July 1, 2001 through June 30, 2004, and the amount of the Agreement shall be reduced by $39,793: $45,793 reduced from the Education & Prevention section and $6,000 increased to the Case Management section of the Agreement, for a new total obligation of $4,738,240.

WHEREAS, this Board has been presented with the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment to the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute the aforesaid Amendment to the Agreement as approved by the County Manager and County Counsel for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
