RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD OF SUPERVISORS TO EXECUTE AN AMENDMENT TO THE GRANT AGREEMENT WITH THE PUBLIC HEALTH INSTITUTE.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the County was funded by the Public Health Institute to create a plan in collaboration with community partners in South San Francisco and the Coastside community;

WHEREAS, the collaborative plan was approved for implementation by the Public Health Institute;

WHEREAS, the Public Health Institute has extended the term through December 31, 2004, with an unchanged agreement total of $645,000; and

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and director to execute said Amendment to the grant Agreement with the Public Health Institute for and on behalf of the County of San Mateo and the Clerk of this Board shall attest the President’s signature thereto.
