RESOLUTION AUTHORIZING EXECUTION OF AMENDMENT I TO AGREEMENTS WITH AVENIDAS, CITY OF MENLO PARK, CITY OF PACIFICA, CITY OF SAN BRUNO, CITY OF SOUTH SAN FRANCISCO, SECOND HARVEST FOOD BANK AND SENIOR COASTSIDERS; AND AMENDMENT II TO AGREEMENTS WITH ALZHEIMER’S ASSOCIATION OF NORTHERN CALIFORNIA, CATHOLIC CHARITIES CYO, CENTER FOR INDEPENDENCE OF THE DISABLED, CITY OF BELMONT, CITY OF DALY CITY, CITYOF SAN MATEO, COASTSIDE ADULT DAY HEALTH CENTER, EDGEWOOD CENTER FOR CHILDREN AND FAMILIES, FAMILY CAREGIVER ALLIANCE, FAMILY SERVICE AGENCY, LEGAL AID SOCIETY, MILLS-PENINSULA SENIOR FOCUS, INC., OMBUDSMAN OF SAN MATEO, INC., PENINSULA VOLUNTEERS, INC., AND SELF-HELP FOR THE ELDERLY

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, this Board has designated itself as the Area Agency on Aging of San Mateo County to carry out programs pursuant to the Older Americans Act;

 

WHEREAS, it is necessary and desirable that the Area Agency on Aging administer the Title III/VII/V and Community-Based Services Programs;

 

WHEREAS, there have been presented to this Board of Supervisors for its consideration and acceptance Amendments I and II, reference to which is hereby made for further particulars, whereby the Area Agency on Aging will provide Congregate Nutrition, Home-Delivered Meals, Brown Bag, Adult Day Care/Adult Day Health Care, Transportation, Senior Employment, Disease Prevention/Health Promotion, Legal Assistance, Ombudsman Programs, Health Insurance Counseling and Advocacy Program (HICAP), Family Caregiver Support Services and Alzheimer’s Programs to the aged, disabled, mentally ill and at-risk youth population of San Mateo County;

 

WHEREAS, all parties now wish to amend the Agreements to continue to provide the Title III/VII/V and Community-Based Services Programs services for Fiscal Year 2004-05 to increase the amount by $2,910,733, for a total obligation of $6,153,014 for the Agreements’ term of July 1, 2003 through June 30, 2005; and

 

NOW, THEREFORE, IT IS RESOLVED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment I to the Agreements with Avenidas, City of Menlo Park, City of Pacifica, City of San Bruno, City of South San Francisco, Second Harvest Food Bank and Senior Coastsiders, and Amendment II to the Agreements with Alzheimer’s Association of Northern California, Catholic Charities CYO, Center for Independence of the Disabled, City of Belmont, City of Daly City, City of San Mateo, Coastside Adult Day Health Center, Edgewood Center for Children and Families, Family Caregiver Alliance, Family Service Agency, Legal Aid Society, Mills-Peninsula Senior Focus, Inc., Ombudsman of San Mateo, Inc., Peninsula Volunteers, Inc., and Self-Help for the Elderly, for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto; and

 

BE IT FURTHER RESOLVED that the Director of Health Services is hereby authorized to execute subsequent Amendments and minor modifications to the Agreements not to exceed the aggregate of $25,000 for the term of each Agreement.
