RESOLUTION AUTHORIZING EXECUTION OF A SECOND AMENDMENT TO THE AGREEMENT WITH ROBERT SHAW AND ASSOCIATES FOR PREPARATION OF ENVIRONMENTAL REVIEW RECORDS (ERR) AND REQUIRED NATIONAL ENVIRONMENTAL POLICY ACT (NEPA) FORMS

 

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the County of San Mateo under Agreement Number 71000-03-C222 entered into an Agreement (“Original Agreement”) to provide a total of $90,000.00 to Robert Shaw & Associates (“Contractor”) for the preparation of Environmental Review Records (ERR) and required National Environmental Policy Act (NEPA) forms; and

 

WHEREAS, the term of the Original Agreement is from 5/15/2003 through 6/30/2005 and the total amount is $90,000; and

 

WHEREAS, it is now the mutual desire and the intent of the parties to increase the total of the Original Agreement from $90,000 to $170,000 for the purpose of completing environmental project reviews and revising the services listed in Exhibit A2; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, a Second Amendment to the Agreement between the County of San Mateo and Robert Shaw and Associates, reference to which is hereby made for further particulars, and the Board of Supervisors has examined and approved the same as to form and content and desires to enter into the same.

 

NOW, THEREFORE, IT IS HEREBY RESOLVED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Second Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

EXHIBIT A2

PROGRAM/PROJECT DESCRIPTION

    I. Description of Services and Contractor’s Responsibilities

A - Preparation of Environmental Review Records (ERR) as required by HUD for all

    CDBG, HOME and ESG funded projects. These records shall include:

B - Prepare HUD National Environmental Policy Act (NEPA) Environmental Review Forms for minor rehabilitation and/or acquisition of existing structures (residential, public facilities and commercial).

 
     

II. Description of County’s Responsibilities:

 

County shall provide technical and clerical assistance to prepare official correspondence from drafts prepared by Contractor.
