RESOLUTION WAIVING THE REQUEST FOR PROPOSAL PROCESS AND APPROVING AN AMENDMENT TO AN AGREEMENT WITH SECURE HORIZONS FOR RETIREE HEALTH COVERAGE FROM JANUARY 1, 2005 THROUGH DECEMBER 31, 2005.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, it is in the best interests of the County and participants in the plan to retain Secure Horizons because its continuation provides affordable coverage with little disruption in doctor-patient relationships; and

WHEREAS, the County desires to approve an agreement with Secure Horizons to provider coverage from January 1, 2005 through December 31, 2005; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into said Agreement,

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors waives the Request for Proposal process and authorizes an agreement with Secure Horizons.
