RESOLUTION AUTHORIZING THE EXECUTION OF A THIRD AMENDMENT TO THE AGREEMENT WITH DELOITTE CONSULTING TO (1) IMPLEMENT ENHANCEMENTS TO ONE-E-APP, PROVIDE FOR MAINTENANCE, AND EXTEND THE TERM; AND (2) AUTHORIZE THE DIRECTOR OF THE HUMAN SERVICES AGENCY TO EXECUTE AMENDMENTS AND MODIFICATIONS NOT TO EXCEED $25,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the Board of Supervisors approved an Agreement with Deloitte Consulting on March 14, 2003, in the amount of $99,000, for the term of March 13, 2003 to June 30, 2003, to initiate the design, development and implementation of the One-e-App, a web-based application processing system that facilitates the processing, collection and storage of data for the Healthy Kids and other locally managed eligibility programs; and

WHEREAS, the Board of Supervisors approved a First Amendment on May 13, 2003 to increase the amount by $343,000, for a total of $442,000, and extend the term to June 30, 2004 for the purpose of adding Phase II, to expand the existing system to include additional managed health programs; and

WHEREAS, the Board of Supervisors approved a Second Amendment on December 16, 2003 increasing the amount by $6,500, for a total obligation of $448,500, to enhance the capabilities of One-e-App by streamlining the re-enrollment process; and

WHEREAS, the parties now wish to further amend the Agreement by increasing the amount by $241,500, for a total obligation of $690,000, to provide: (1) Implement Healthy Kids enhancements to the One-e-App program; (2) Develop and implement an interface to the Welfare Case Data System; and (3) Extend the maintenance and Agreement term to July 31, 2005; and

WHEREAS, the parties also wish to authorize the Director of the Human Services Agency to execute amendments and modifications not to exceed $25,000; and

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Third Amendment to the Agreement for and on behalf of the County of San Mateo; and the Clerk of this Board shall attest the President's signature thereto.

BE IT FURTHER RESOLVED, that the Director of the Human Services Agency shall be authorized to execute subsequent amendments and modification not to exceed $25,000.
