RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO 1) WAIVE THE REQUEST FOR PROPOSAL PROCESS TO DEVELOP A UNIFORM ASSESSMENT TOOL; 2) EXECUTE AN AGREEMENT SUPERSEDING AGREEMENT NUMBER 57000-05-D013 WITH C.H. MACK, INCORPORATED FOR THE PROVISION OF AN AUTOMATED CASE MANAGEMENT SYSTEM FOR CASE MANAGEMENT PROGRAMS IN AGING AND ADULT SERVICES AND 3) AUTHORIZE THE DIRECTOR OF HEALTH SERVICES TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an agreement superceding Agreement Number 57000-05-D013 dated October 13, 2004 and approved by the Director of Health Services, reference to which is hereby made for further particulars, whereby, C.H. Mack, Inc., will provide an automated case management system for specific programs in Aging and Adult Services, including the automation of the InterRAI-HC uniform assessment tool;

WHEREAS, it is in the best interest of the County to waive the Request for Proposal Process for development of the InterRAI-HC uniform assessment tool within an automated case management system; and

WHEREAS, both parties now wish to enter into said Agreement to provide a case management system and to develop the InterRAI-HC uniform assessment tool within the Q Continuum System for the period of October 1, 2004 through September 30, 2005, for a total obligation of $110,940:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Request for Proposal process is waived; and the President of this Board be, and is hereby authorized and directed to execute said Agreement superseding Agreement Number 57000-05-D013 dated October 13, 2004 for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto; and

BE IT FURTHER RESOLVED that the Director of Health Services is hereby authorized to execute subsequent Amendments and minor modifications to this Agreement not to exceed the aggregate of $25,000 and to make minor changes in the types of services and activities provided under the agreement, for the term of the Agreement, provided the additional amount is within budgetary appropriations and the term is not extended.
