RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH INNVISION - THE WAY HOME, AND AUTHORIZING THE DIRECTOR OF THE HUMAN SERVICES AGENCY TO EXECUTE SUBSEQUENT AMENDMENTS TO THE AGREEMENT, AND TO MAKE MINOR MODIFICATIONS IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED BY INNVISION - THE WAY HOME UNDER THE AGREEMENT, THE CUMULATIVE COST OF WHICH AMENDMENTS AND MODIFICATIONS IS NOT TO EXCEED $25,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, Clara-Mateo Alliance Inc., merged with InnVision - The Way Home under the new organizational structure Clara-Mateo Alliance Inc., was dissolved as a corporation and became a division of InnVision – The Way Home; and

 

WHEREAS, the County now wish to enter into an Agreement with Contractor, to provide for the Elsa Segovia Center and for emergency and transitional shelter services for a total obligation of $118,263, of which $80,000.00 from CDBG funds, $37,363 from CalWORKS funds, $900 Center for Substance AbuseTreatment (CSAT) funds, for the period of 11/1/2004 to 10/31/2005; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, an Agreement between the County of San Mateo and InnVision – The Way Home (“Contractor”), reference to which is hereby made for further particulars, and the Board of Supervisors has examined and approved the same as to form and content and desires to enter into the same; and

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED, that the Director of the Human Services Agency shall be authorized to execute subsequent amendments to the Agreement, and to make minor modifications in the types of services and activities provided by Contractor, under the Agreement, the cumulative cost of which amendments and modifications is not to exceed $25,000.
