RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH THE SAN MATEO HEALTH COMMISSION, DBA THE HEALTH PLAN OF SAN MATEO, FOR THE MENTAL HEALTH SERVICES DIVISION TO ADMINISTER THE MENTAL HEALTH BENEFITS AND PROVIDE MENTAL HEALTH SERVICES FOR BENEFICIARIES OF THE HEALTHY FAMILIES PROGRAM, THE HEALTHY KIDS PROGRAM AND THE HEALTHWORX PROGRAM

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby Mental Health Services shall administer mental health benefits and provide mental health services; and

WHEREAS, both parties now wish to enter into an Agreement, whereby Mental Health Services shall administer the mental health benefits and provide mental health services for the Healthy Families Program, the HealthWorx Program, and the Healthy Kids Program from the date of approval by this Board through September 4, 2006, wherein payments shall be made on a fee-for-service basis with no net county cost; and

WHEREAS, this Board has been presented with the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby, authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
