RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH SEQUOIA HOSPTIAL AND MEDICAL CENTER AND AUTHORIZING THE DIRECTOR OF THE HEALTH SERVICES AGENCY TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED THE AGGREGATE OF $25,000 AND TO MAKE MINOR CHANGES IN THE TYPE OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement with Sequoia Hospital and Medical Center, reference to which is hereby made for further particulars;

WHEREAS, this Board has been presented with the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement;

WHEREAS, both parties now wish to enter into an Agreement, whereby Sequoia Hospital and Medical Center shall provide acute inpatient psychiatric services for adult Medi-Cal and uninsured clients of San Mateo County for the period July 1, 2004 through June 30, 2005 for a maximum obligation of $882,411;

WHEREAS, the collective maximum amount for all inpatient hospital agreements for FY 2004-05 is $882,411;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby, authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED, the Director of the Health Services Agency is authorized to execute subsequent amendments and minor modifications not to exceed the aggregate of $25,000 and to make minor changes in the type of services and activities provided under the Agreement.
